A Comparative Study between Rnfl Using Oct and Iop Changes in Primary Open Angle Glaucoma Patients Undergone Trabeculectomy in the Sub- Himalayan Region

Abstract

Purpose:  To study the relation between optical coherence tomography measurements of RFNL and IOP in a population of clinical patients with primary open angle glaucoma undergone trabeculectomy pre-op and 1week, 1 and 3 month post-operatively in the Sub-Himalayan region.


Methods: Normative data was collected from 27 patients having primary open angle glaucoma. The patient population consists of adults with open anterior chamber angle and with demonstrative RNFL damage, with corresponding visual field loss. Average and quadrant RNFL thickness measurements were taken. 3D OCT1MAESTRO was used to measure quadrant RNFL thickness within a week before surgery and at 1 week, 1 and 3 month post operatively. Intra ocular pressure is measured by using a Goldman‘s applanation tonometer within one week pre-operatively and 1 week, 1 and 3 month post-operatively. Pre and post operative value were analyzed using Repeated Measure ANOVA with Bonferroni correction.


Results: Intra Ocular Pressure (IOP) in pre-operative period was found to be 26.93 ± 2.786 and decreased in the post-operative 1week to 9.07± 3.43 (p ˂ 0.05). 1 month 11.41± 4.254 (p ˂ 0.05) and in the 3month was 11.81± 3.552 (p ˂ 0.05) after trabeculectomy. Significant increase in Inferior RNFL thickness was observed 1 week post-op, but after 3 month reverted back to pre-operative values. RNFL thickness had no correlation with IOP changes after trabeculectomy.


Conclusions: Short term fluctuations were noted in the RNFL thickness and IOP, post-operatively following Trabeculectomy. RNFL thickness showed improvement in 1 week post-operatively only, but reverted back to pre-operative values at 3 month. IOP changes showed significant decrease in post-operative period, but the changes had no correlation with RNFL thickness.
